Jaimie Machado, APRN, FNP-C, PMHNP-BC

Jaimie Machado, is the founder of PATH: Personalized Approach to Health, LLC, and an independent advanced practice registered nurse with over 20 years of experience in healthcare. With a career rooted in both physical and mental health care, Jaimie is dedicated to providing comprehensive, patient-centered support that honors each individual’s unique health preferences, values, and beliefs.

Jaimie’s passion for preventative health was sparked during her early years working in the intensive care unit, where she witnessed firsthand the critical need for proactive approaches to wellness. In 2017, she earned her degree as a Family Nurse Practitioner and began serving in community health, providing essential care to underserved populations. Recognizing the growing demand for mental health services, she pursued further specialization and completed her post-master’s degree as a Psychiatric Mental Health Nurse Practitioner in 2023.

Throughout her career, Jaimie has developed extensive experience treating a wide range of conditions, including chronic fatigue, cardiovascular disease, substance use disorders, anxiety, depression, post-traumatic stress disorder, and bipolar disorder. Her approach is deeply integrative and compassionate, focusing on the whole person and empowering individuals on their journey to wellness.

Outside of her professional work, Jaimie is a proud mother of two and enjoys an active outdoor lifestyle with her three dogs. She finds joy in skiing, hiking, and kayaking, embracing every opportunity to connect with nature.

Maria Slajda, MSN, FNP-C – Board-Certified Family Nurse Practitioner

Maria Slajda is a highly skilled and compassionate Family Nurse Practitioner with 18 years of diverse experience in the healthcare field. Maria began her nursing career at Hartford Hospital, where she spent 11 years specializing in cardiothoracic surgery critical care. These formative years provided her with a solid foundation in acute care and helped shape her deep passion for preventative health.


In 2018, Maria completed her Master of Science in Family Practice at the University of Saint Joseph, expanding her expertise to provide comprehensive primary care. Over the following five years, she worked in community health in Hartford, offering primary care services while integrating mental health services. She has treated a wide range of medically complex patients including those managing chronic conditions such as hypertension, diabetes, obesity and HIV. Maria is certified as an HIV prevention provider. She has experience in medication-assisted treatment (MAT) for individuals struggling with substance use disorders, helping patients manage and recover from addiction.

Maria’s background also includes working in a busy urgent care office, where she honed her skills in handling acute, immediate health issues in an outpatient setting. This experience reinforced her ability to make quick, informed decisions in fast-paced environments. Maria has a growing passion about women’s health, including hormonal replacement therapy. She provides holistic, patient-centered care for women, especially those navigating life transitions, managing hormonal changes, and healing from past trauma. Her approach incorporates a deep understanding of the emotional and psychological aspects of health, ensuring that each patient feels safe, heard, and respected throughout their care journey.


As a bilingual nurse practitioner, Maria is dedicated to bridging communication gaps for monolingual Spanish-speaking patients. Her ability to provide culturally competent care ensures that all patients, regardless of language or background, receive the respect and understanding they deserve. Born and raised in Peru, Maria deeply values cultural wisdom and continues to explore her native roots to integrate holistic and traditional healing practices into her care.


Maria’s patient-centered philosophy focuses on partnership and empowerment, helping all individuals manage their health, improve their quality of life, and achieve long-term wellness. Whether working with patients with chronic conditions, women’s health issues, or those seeking preventative care, Maria is committed to offering compassionate, culturally sensitive, and holistic care tailored to each person’s needs.

Jordan Wasik, LPC

Jordan Wasik is a Licensed Professional Counselor with over a decade of experience providing compassionate, evidence-based care in the field of mental health. She earned her Bachelor’s degree in Psychology from Salve Regina University in 2012 and went on to complete her Master’s degree in Clinical Mental Health Counseling at the University of Saint Joseph in 2015.

Jordan began her career at Wheeler Clinic in Hartford, Connecticut, where she worked with individuals experiencing mental health challenges, substance use disorders, and co-occurring conditions. During her seven years at Wheeler Clinic, she served in several roles, including leading the Intensive Outpatient Program (IOP) for women, completing intake assessments, and providing individual therapy. She also worked for the Bettor Choice Gambling Program, offering support to adults struggling with gambling addiction. Jordan has received specialized training in Eye Movement Desensitization and Reprocessing (EMDR), gambling addiction, military populations, first responders and animal-assisted interventions. In 2019, she opened her own private practice in Canton, CT, where she provided individualized and integrative mental health services until 2025. After years of private practice, Jordan made the intentional decision to close her business to prioritize work-life balance—an example of the self-care principles she encourages her clients to embrace.

Jordan has maintained a longstanding collaborative relationship with PATH and is proud to join the organization as the next step in her professional journey. Her therapeutic approach is person-centered, culturally competent, and deeply rooted in the belief that growth and healing occur through authentic therapeutic connection. She works with children (ages 5 and up), teens and adults. A distinctive aspect of Jordan’s therapeutic approach is her partnership with Remington (“Remi”), her Golden Retriever. Together, they provide animal-assisted therapy, offering clients a comforting and supportive environment. Remi follows in the paw prints of Jordan’s first therapy dog, Tessa, and is currently working toward his therapy dog certification. His presence brings warmth, empathy, and acceptance to every session. Outside of her professional work, Jordan lives in Canton with her husband and their two young children. She enjoys fitness, family time, training with Remi, and exploring the scenic beauty of the Farmington Valley.

Sir Remington Braveheart (“Remi”), CGC Therapy Dog In Training

Sir Remington Braveheart, affectionately known as Remi, is a four-year-old purebred Golden Retriever and a key member of the therapy team. Remi comes from a long line of accomplished Golden Retrievers known for their work in hunting, search and rescue, and service roles. As a field Golden, Remi is distinguished by his deep red coat, shorter hair, and signature white chest marking. From the very beginning, Remi was hand selected by his breeder for his social temperament and natural aptitude for therapy work. He joined Jordan’s family at just nine weeks old and has been lovingly raised and socialized around adults, children, and other animals. Through intentional desensitization and ongoing training, Remi is comfortable in a wide range of environments, equipment, and situation —making him a reliable and soothing presence in the therapy office. Remi has earned several American Kennel Club (AKC) achievements, including Star Puppy, Trick Dog, and Canine Good Citizen (CGC) certifications. He continues to participate in advanced obedience and therapy preparation training alongside Jordan. Together, they are working toward official therapy dog certification, which evaluates obedience, temperament, and teamwork—ensuring that both handler and dog provide a calm, safe, and positive experience for clients. In the therapy setting, Remi offers gentle comfort, empathy, and unconditional acceptance. His joyful spirit and attuned nature, often help clients feel more at ease and supported during sessions. When he’s not at work, Remi enjoys spending time with his “brother,” a two year-old rescue dog from Aruba. He loves swimming, chasing tennis balls, and learning new tricks. Above all, Remi is happiest when he is by Jordan’s side—ready to share his

warmth and wagging tail with everyone he meets.
